Abstract

Stunting remains a persistent public health concern in Indonesia. In Taman Ayu Village, West Lombok, 105 stunting cases were recorded in 2022. Exclusive breastfeeding success, initiated through antenatal breast care and community support, is a critical intervention for stunting prevention. A needs assessment revealed inadequate maternal knowledge, sociocultural rejection of colostrum, and absence of trained community cadres to support breastfeeding preparation. This community empowerment program aimed to enhance cadres’ knowledge and skills in antenatal breast care and breastfeeding management among third-trimester pregnant women. A six-month training and mentoring program (March–August 2025) was implemented involving 20 cadres and 10 pregnant women. The intervention consisted of structured education, breast-care demonstration using phantom models, guided practice, mentorship, and pre-post evaluation. Knowledge levels were measured using validated questionnaires, while breastfeeding preparation skills were assessed via performance checklists. Findings indicated a substantial improvement in cadre capacity. Pre-test results showed 85% of cadres had low knowledge, increasing to 100% in the good category post-intervention. Skill competence improved from 10% to 100% proficiency following training. The experiential learning approach, direct modeling, repetitive practice, and structured feedback significantly contributed to skill acquisition and self-efficacy. Output achievements included mass-media dissemination, social media health campaigns, and Intellectual Property Rights (IPR) registration for educational leaflets and video media. To ensure sustainability, the program initiated the formation of a community breastfeeding cadre group, integration with primary health center supervision, peer mentoring by senior cadres, and policy advocacy toward a Breastfeeding-Friendly Village regulation. The intervention demonstrates that capacity building of local health cadres strengthens community readiness for early breastfeeding adoption and supports sustainable stunting prevention strategies.
